Benevolence Form

1. Approval Process:
1. Request needs to come to the Region Leader
2. Region Leader, then Super Region Leader determines if request is valid, then requests approval from Tim Kernan.
2. This form will not be accepted without a social security number (SSN).
3. Need to have the requestor’s signature.
4. Need to have ministry approval’s signature; only Tim Kernan can sign.
   If one of the signatures is missing, the form will be returned to you as incomplete.
